What do I need to bring with me to my tax appointment?

You will need to bring the following with you to your tax appointment:
 
  • Copy of last years tax return
  • Valid Picture ID for Taxpayer & Spouse (Driver's License, State ID, Passport)
  • If Married and filing jointly, both spouses must be present to sign
  • Social Security Cards or Individual Taxpayer Identification Numbers (ITIN) for ALL family members
  • Birth dates for you, your spouse and dependents on the tax return
  • Wage and earning, pension or other retirement income statements (Forms W-2, W-2G, 1099-R, 1099-Misc) from all employers/payers
  • Interest and dividend statements from banks (Forms 1099-INT and -DIV)
  • 1098 or real estate tax receipt
  • Proof of unemployment compensation
  • Brokerage statements
  • A copy of a voided check or bank account and routing numbers for direct deposit
  • Proof of child care expenses
  • Proof of Health Insurance (Forms 1095-A, B or C, Affordable Health Care Statements) or exemption certificate
